The Canadian Cardiac Randomized Evaluation of Antidepressant and Psychotherapy Efficacy (CREATE) is in progress with patients randomized to four arms. Patients either received citalopram (Celexa), and inter¬personal psychotherapy, placebo and interpersonal psycho¬therapy, routine clinical management, or routine clinical management and placebo. Other agents in this group include fluoxetine (Prozac), the serotonin antagonist mirtazapine (Remeron), and the norepinephrine- and dopamine-reuptake inhibitor bupropion (Wellbutrin). These drugs may cause significant increase in blood pressure and should be avoided in patients with hyper¬tension or ischemic heart disease manifested by angina or heart attack.
Serotonin antagonists and reuptake inhibitors: nefa-zodone (Serzone), trazodone (Desyrel). These agents The bicyclic venlafaxine (Effexor) may cause hypertension, Serotonin and norepinephrine reuptake inhibitors. Tricy-clic antidepressants (amitriptyline, imipramine, doxepin, desipramine). These older antidepressants are well known to have adverse cardiac side effects and are contraindicated in patients with heart disease.
